We are now offering telehealth services!

 

HOW DO I SCHEDULE A TELEHEALTH APPOINTMENT?

  • To schedule a telehealth appointment with a medical provider, call 801-429-2000 and a receptionist will be happy to schedule your visit.

  • You will then receive a welcome email with a username and password. (If you happen to receive two emails, please use the password from the email that was received most recently.)

  • The welcome email will provide an option to download the mobile Qure4u app. Please download the app, login, and test your connection as soon as possible.

  • Please enter the username and password provided in the email. Do not register as a new user. You will have the option to change your password after logging into the app.

 

HOW DO I CHANGE THE LANGUAGE TO SPANISH?

  • To change the language to Spanish, click on the three-bar icon in the top right corner of the screen. The fifth option  (Language/Idioma) will allow you to change the language to Spanish.

 

I HAVE LOGGED INTO THE TELEHEALTH APP.  IS THERE ANYTHING I NEED TO DO BEFORE MY APPOINTMENT?

  • Yes. Note that the home page is the heart icon on the bottom of the screen. When you click this icon, the following boxes should appear: Update Demographics, Pay Copay, Sign Privacy Notice, Sign Assignment of Benefits, Sign Release of Information and Sign Telehealth Consent Form. Please complete each of these documents. A green checkmark will appear next to the box once it is complete.

    • If you have insurance with a copay, you will need to enter your card information into the app. If you do not have insurance, simply click through this section.

HOW DO I BEGIN MY TELEHEALTH APPOINTMENT?

  • Once you are logged into the app, click the bottom right icon which is the shape of a clock. This will show all your scheduled appointment times. Click the appropriate appointment time.

  • Click “Start video call here.

  • You are ready to go!

HOW WILL PAYMENT WORK?

The price for your telehealth visit will be the same as an in-office visit. Call 801-429-2000 with any questions.
