REPORTS TO:  Medical Assistant Supervisor



WORK HOURS/TRAVEL REQUIREMENT:  Typical work week, with some Saturday and evening shift work.  Travel is infrequent and typically local (within 50 miles).

EDUCATION REQUIRED: High school diploma or GED, MA School Completion

EXPERIENCE REQUIRED:  Requires completion of all externship hours.





The Medical Assistant is assigned to work with a particular provider. He/she will be accessible to and complete all assignments as given by provider such as perform blood draws, vital signs, laboratory specimen work, assist provider with procedures, chaperone provider when needed, and interpret for provider as needed. He/she is responsible to complete assigned duties in the maintenance of clinical areas and supplies and will correctly documents this information. The Medical Assistant will perform other necessary duties required by the Medical Assistant Supervisor, Site Manager, or management team to achieve the goal of quality providing primary health care to MCHC patients.


The Medical Assistant assumes responsibility for follow up on messages and labs for designated provider on a daily basis; immediate attention will be given to urgent messages/labs.  All pharmacy requests and messages will be called in or answered by the end of shift. He/she will be responsible to identify them self, greet patients, and answer telephones in a courteous, timely, and professional manner. He/she will also be responsible to assist in scheduling appointments for patients as needed.


The Medical Assistant is responsible to explain all Medical Assistant procedures (e.g.: clean urine catch) to patients in a correct and easily understood manner. He/she is responsible to correctly enter all encounters with a patient into the corresponding EMR chart.  He/she will assure all messages and correspondence is placed in the EMR chart in a timely manner.


The Medical Assistant is responsible to correctly perform procedures according to protocols such as vital signs, lab tests, etc. He/she is also responsible for maintaining current Basic Life Support certification, required immunizations, and other certifications. He/she may be responsible for additional duties as assigned by the Medical Assistant Supervisor, Site Manager and/or any member of management team as needed.


MCHC is a Patient Centered Medical Home. This includes all staff members working as a team alongside patients to achieve a higher quality primary care experience. This position is integral to this process.


This position also is responsible for any and all other task that may be assigned to him/her by their supervisor that has not been described in their job description.

The Mental Health Therapist is responsible for providing diagnostic assessments, therapy, counseling, and support services for a diverse caseload including adults, adolescents, and families experiencing mental health issues, communication challenges, or family conflict. They are a deeply familiar with other services and resources within the community and work closely with them to provide information and support when required outside of MCHC.



Education or Formal Training (Licenses/Certifications):

  • LCSW, CMCH, MFT, licensed in the State of Utah required.

  • Master’s Degree from accredited social work program required.

  • CSW also acceptable if working toward licensure, Supervision Provided


  • Two (2) years minimum experience preferred.

  • An equivalent combination of education and experience may be substituted.

  • Prior experience with Electronic Medical Record and/or practice management systems.

  • Excellent interpersonal, communication, customer service, and compassionate patient skills required with strong leadership, analytical and organizational skills.


  • Bilingual: English/Spanish, strongly preferred.

Essential Duties/Responsibilities




Provides comprehensive care counseling and makes referrals to appropriate agencies or providers. Works with patients in tandem with MCHC’s Medically Assisted Treatment (MAT)/substance abuse program.  Serves as a client advocate to coordinate required services or to resolve emergency problems in crisis situations.

Provides services in compliance with laws, regulations and JCAHO standards. Demonstrates competency documenting signs of abuse and/or neglect, ensuring both are reported to authorities as required. Refers patients to medical providers for medication evaluation, pain assessment and treatment, as necessary.  Collaborates with the Mental Health team on items such as crisis intervention, DSM system diagnosis, and individual/family/couple therapy. Employs interdisciplinary cases in conjunction with MCHC medical staff.

Performs all duties, responsibilities and competencies in a manner that is effective and appropriate to the age group(s) of the patients to whom care is being provided. Provides assessments that are consistent with the age group and population involved, demonstrates full understanding and identifies mental health issues that are lifecycle related. Utilizes therapeutic techniques that are age appropriate, provides individual, group, and family counseling sessions, etc. Provides psycho-educational services and assists patients in linking with charity care providers and community resources by building referral sources. Consults with medical providers regarding patient’s mental health issues and progress. Prepares psychotherapy documents encompassing all patient care and ensures mental health forms are complete and accurate.


Develops and implements treatment plans based on clinical experience and knowledge, conducts initial family assessments, and refers patients, clients, or family members to community resources or to specialists as necessary.


Provides written therapy-based evaluations and/or letters as required for social security, school, immigration, court documents, support animals, etc.


Participates in marketing mental health services, attends health fairs and other special events and speaking engagements on behalf of MCHC. Maintains updated community resource and information file and coordinates special projects to enhance overall patient care. Attends Continuing Education courses as needed and presents information to other staff.


Performs other duties as assigned.

Education or Formal Training (Licenses/Certifications):
• High school diploma or GED required; for dental responsibilities: dental assistant or x-ray certification preferred.

• One (1) year experience with customer service tasks preferred.
• Computer skills and software aptitude required.
• Communication and strong compassionate customer service skills required.
• Multi-line phone proficiency required.
• Insurance coding and medical or dental terminology preferred.
• Ability to maintain confidentiality, patient records, manage multiple schedules, and ensure patient information is complete and up to date is required.

Greets and welcomes patients, performs all check-in tasks including verifying income, updating patient information (including updating all forms related to HIPAA, Patient Responsibility, etc.), collecting copays, and guiding patients to the waiting area. May also need to assist patients with the information forms. Responsible for answering phone calls to schedule, reschedule, schedule follow-up appointments, provide reminder calls (includes leaving applicable messages), and manage canceled appointments. Is responsible for verifying methods of payment and collecting payment as needed.

Responsible for calling insurance companies to verify insurance and determine if patients are required to make co-payments.

Makes sure that the insurance information is updated in the chart correctly.

Creates patient case records (e.g., current medications, clinical questions, etc.).

Prepares patients’ charts and daily schedules for dental staff (if applicable) and performs general office duties, such as
answering phones, photocopying, filing, faxing documents, sending medical and/or dental records, stock office supplies, and keeping the front desk areas clean and organized.
