Health Fair 2021 Registration

While we are unable to host our annual in-person Community Health Fair this year, Mountainlands Community Health Center will still be providing backpacks for school-age children. Children ages 3-4 will receive a free sling bag. Children age 5 and older (K -12) will receive a free backpack with school supplies.

Please complete this form to register for a 10-minute time slot to meet with our staff and receive your backpack! As a reminder, each child must be present at the appointment in order to receive the free backpack and school supplies.
