Health Fair 2021 Registration
While we are unable to host our annual in-person Community Health Fair this year, Mountainlands Community Health Center will still be providing backpacks for school-age children. Children ages 2-4 will receive a free sling bag. Children age 5 and older (K -12) will receive a free backpack with school supplies.
Please complete the form below to register for a 10-minute time slot on August 14th, 21st, or 28th to meet with our staff and receive your backpack.
Each child must be present at the appointment in order to receive the free backpack and school supplies.
