| MCHC has some patients who are fully covered by private health insurance, CHIP (Children's Health Insurance Program), Medicaid and/or Medicare. A large share of our patients are self-paying, uninsured patients. Patients who have insurance are requested to use their medical insurance cards. They will then pay either the insurance co-payment or our sliding fee, whichever is less. |
| In order to make health care affordable to people at lower income levels, we have instituted a sliding fee schedule based on your income level and the size of your family. To become eligible for this reduced fee schedule, you need to talk with our staff and provide documentation concerning your income. The interview process does not take very long and is confidential. No one is denied service due to an inability to pay. If you have difficulty making your co-payment, ask to see one of our Billing clerks. |
| Medical and dental clinics normally ask for payment up front. This helps keep bookkeeping costs low. The payment that MCHC asks for is often only a small portion of the full fee for the service. Under unusual circumstances, the Billing staff can authorize a one-time-only delay of your payment. This delay is available only to established patients and those who have already qualified for reduced fees. |
| Two bilingual (English/Spanish) Medicaid Eligibility Workers are available on-site to assist our clients with applications for Medicaid, CHIP (Children's Health Insurance Program), Disability and related benefits.
The Utah Farm Workers Health Program maintains an active presence at our Clinic. Eligibility Specialists are available for all seasonal and migratory workers, as well as farm workers. |
| MCHC will perform minor elective surgical and nonessential (i.e. birth control) procedures. The fees for these items may be higher than for essential medical services. Sliding fee schedules may still apply. Payment is due at the time of service-payment for these services may not be deferred. |
| Yes. Mental health services have been available on an ability to pay basis since January 1997. |
| No. MCHC has no funds to pay for services that patients receive someplace else. This includes referrals to hospitals, pathology, laboratories, X-ray facilities, etc. MCHC can not guarantee they will provide free services or services at reduced fees. You need to discuss your situation with these other facilities and meet their guidelines in order to qualify for any assistance. |
| No. MCHC does not have a fully-equipped medical laboratory, nor do we have funding to cover tests for other providers. |
| Because the demand for dental services is so high, we schedule only pre-qualified new patients who pay a $40 deposit. To pre-qualify, you need to be eligible for our services and register with the Dental Receptionist (429-2012). Emergencies will be worked in as the schedule permits. |
- Come into the clinic and fill out the "Patient Registration Form" for new patients. You may print this form out and fill it out.
- We accept "Patient Registration Forms" at the front desk Monday through Friday 10 AM to 4 PM.
- If you have insurance, bring your insurance card.
- If you do not have insurance, bring in verification of income for the past month. This may include:
- Last months check stubs
- State unemployment funding information
- State disability funding information
- A letter from whoever is supporting you if you are not receiving any type of income
- Last year's tax return or tax statement
- Make an appointment to be seen.
- New patients can usually schedule an appointment one to two weeks after registration; however, once you are seen by one of our providers, your immediate family becomes "established". This means that if someone in your family gets sick, more appointment slots will be available to you and your immediate family, and you will most likely get an appointment for the same day or next morning. Therefore it would be wise to register at Mountainlands and to be seen by one of our providers before someone in your family gets sick.
