The Family Dental Plan is State-sponsored; but self-funded. It's purpose is to serve Medicaid and CHIP patients of all ages by providing access to gentle, quality dental care.

The family Dental Plan is very proud of its staff. Each member is highly qualified and sensitive to the needs of our patients. When you come to your appointment, you will find them welcoming and friendly. Our dentists and support staff understand that you may have special problems or conditions, and they are pleased to help and accommodate you.

In order to receive dental care from the Family Dental Plan, you must be eligible for Medicaid, CHIP, or earn less than 200% of the poverty level. You can not be signed up with another dental program which requires you to go to a particular dentist. Then contact your nearest Family Dental Plan clinic for an appointment. We are usually open from 7:00 a.m. to 5:30 p.m., Monday through Friday, except public holidays.

• General dental services covered by Medicaid, including fillings, root canals, and surgeries.
• Same day appointments for relief of pain. Special care to help the very young and the very nervous fell comfortable.
• Referral for specialty conditions and services beyond the scope Medicaid and Family Practice.
• Dental health education to help you and your family have great-looking smiles.
• Toys, videos, and books to entertain the children (and adults) while they wait, and special treasures when the appointment is over.
• Conformance to quality programs to make sure that the dentistry you receive meets high standards.
• Strict disinfection and sterilization procedures to protect all of our patients.
• Specialists available CHIP provider.
• Special rates for those people who earn less than 200% of poverty level.
